    4. PTSD checklist
    5. Clisch clinician survey
    6. Beck dep/anxiety
    7. Beck mood
    8. Danger assessment scale
    9. Life events checklist
    10. IPV measures
    11. Abusive observation scale
    12. TAG
    13.TSI
    14. PAI (test for exaggeration and malingering)

    Please note that “survey”, “scale”, “measures”, “inventory”, “checklist” and “questionnaire” are ALL self-report by patient. These are not considered sufficient for formulating DSM-V diagnoses. In order to make a definitive diagnosis, the patient (especially in a forensic case) should has a full battery of Neuro-psychological testing, including MMPI-2 and CAPS

    Finally, of note is that Dr. Hughes did not evaluate AH (either by self-report or other) for ETOH, Rx or illicit drug use/abuse. In one early testimony by Dr. Kipper and nurse there was communication about AH being on an anti-psychotic medication. Quetiapine. Dr. Kipper stated that AH needed a “higher dose” to control her behavior and ordered nurse to increase the dosage given ( I am fairly certain this communication b/w Kipper and RN occurred in Australia, after JD had his fingertip slice off). This information is crucial in order to provide a more complete/authentic evaluation and diagnostic impression of AH. Dr. Hughes failed to do so
